What do you need to do once you have read the policy and wish to set up access
Ensure you read the online appointment policy. Once you understand the policy you will need to come into the practice and ask a receptionist to register you for online access. The receptionist will ask for photographic ID.
Acceptable forms of photographic ID are a passport or photo drivers licence.
You will then be handed a document containing your username and password (which you can change when you log on).
We cannot set patients up over the phone. Online access allows access to some of your medical information and we have to ensure appropriate protocols are in place to protect access to your medical information.
Each patient will need to set up their own, specific online access. Parents and guardians can set up linked access for their children under the age of 16. We may ask for additional information in order to set this up.
